Refer a Patient

UPT Services makes it easy for physicians, discharge planners, case managers, home care agencies and other providers to get patients the physical therapy they need, at home. Dr. Oleg Uzhansky, PT, DPT, CAPS, treats patients one-on-one across Northeast Philadelphia, Bucks County and Montgomery County.

Two ways we can see your patient

1. In-home outpatient physical therapy (Medicare Part B)

For patients who would benefit from therapy at home but don't need a home health agency. UPT Services bills directly. We accept:

  • Medicare Part B
  • Aetna Medicare plans
  • Independence Blue Cross plans, including Keystone 65

The patient does not need to be homebound. Standard Part B cost-sharing applies (usually 20% after the deductible), and many supplement plans cover that share.

2. Home health and home care, through our partner agencies

For homebound patients who qualify for home health (Medicare Part A), or who also need skilled nursing or home care. Dr. Uzhansky is a contracted physical therapist for several home health and home care agencies in the area. We can work with the patient's current agency or connect them with one of our local partners, so therapy and nursing follow one plan of care.

Not sure which applies? Send the referral anyway. We'll check the patient's coverage and point them to the right option.
