Home Health & Skilled Nursing Coordination

Sometimes physical therapy is only part of what you need at home. UPT Services works alongside local home health agencies, so if you also need skilled nursing care, we can coordinate with the agency you already have or connect you with one we work with.

How we work with home health agencies

Dr. Oleg Uzhansky, PT, DPT, serves as a contracted physical therapist for home health agencies across Northeast Philadelphia, Bucks County and Montgomery County. That means we already know the nurses and care teams in the area and how to work with them, so your therapy and your nursing care move in the same direction.

If you already have a home health agency

  • We talk to your nurse. We share progress and concerns with your agency's care team.
  • One plan, not two. Your therapy goals line up with your nursing plan of care.
  • Fewer surprises. Changes in your balance, strength or safety at home get passed along quickly.
  • Family stays informed. Caregivers hear the same message from everyone.

If you need nursing care and don't have an agency

If we see during a visit that you may need skilled nursing, we'll tell you and your family, and with your permission we can connect you with a local home health agency we work with. The choice of agency is always yours.

When skilled nursing at home may help

  • Coming home after a hospital or rehab stay
  • Recovering from surgery
  • Wound care
  • Help managing medications
  • A new diagnosis or a change in a long-term condition
  • Monitoring ordered by your doctor

Your doctor decides whether skilled nursing is medically necessary and orders the care. We help get the conversation started.

What to expect

  1. Tell us what's going on. Call (215) 760-8229 and let us know whether you already work with an agency.
  2. We coordinate. We reach out to your agency, or help you find one, and involve your doctor.
  3. Care at home. Your therapy and nursing visits happen where you live, with everyone working from the same plan.

Frequently asked questions

Does UPT Services provide nursing care?
No. We provide physical therapy. Skilled nursing is provided by a licensed home health agency. Our role is to coordinate with that agency or help you connect with one.

Do I have to use an agency you recommend?
No. You are free to choose any agency, and we'll coordinate with whichever one you pick.

How is this covered by insurance?
Coverage depends on your plan and on the care your doctor orders. We'll help you understand how therapy and nursing are covered in your situation before care starts.
